This is a new service of SDKC so please take a moment to understand how it works before you begin.
 
1. Complete the registration form. Select if you are going to pay online and list the name on the credit card. This is to assure you will be properly credited with your payment. When you submit you will be taken to a thank you page.
If you have another registration to complete click on the link "Click HERE to return to the registration page" and submit your additional registrations.
 
2 If you wish to pay your fees online you can select the sport, enter how many registrations you are paying for and enter the names of the child or children. Again this is to assure you are properly credited with your payment. Click on the Add to Cart button and will be taken to PayPal where you will enter you information to pay your fees online.
If you are registering more than two children in the same sport make your payment at the kids club to receive the discount given for three or more.

I, the parent guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the Soddy-Daisy Kids Club. I assume all risks and hazards incidental to such participation including transportation to and from the activities and do herby waive, release, absolve, indemnify and agree to hold harmless the parent, local league, ruling body, organization, the organizers, sponsors, supervisors, participants, and the persons transporting the said registrant to and from activities from all arising out on injury to them. CONSENT FOR MEDICAL TREATMENT: As the parent or legal guardian of the above named applicant, I herby give consent for emergency medical care prescribed by a duly licensed Doctor of medicine or dentistry. This care may be given under whatever conditions are necessary to preserve the life or well being of my dependent.:
